For 30 years, School Health Clinics of Santa Clara County has offered low-cost, high-quality medical services to low-income families.
Our mission is to provide high-quality, affordable primary medical care for the low-income community.
Research has shown that providing low-income and minority students with healthcare and health education give kids a chance to stay in school and perform better academically.
Although we began as pediatric clinics, we now extend our services to adults.
No one has ever turned away due to their inability to pay.
All five clinics are located on school grounds in Santa Clara County, right in the neighborhoods where our patients live.
Our staff is bilingual and multicultural so our patients can easily understand the medical advice that is given and is not intimidated because of a language barrier.
*SUMMARY OF JOB FUNCTION* The Outreach and Enrollment Specialist will work to provide outreach opportunities for community residents who lack access to healthcare and health insurance, provide enrollment and application follow-up assistance and build/strengthen enrollment collaborations with local partner organizations.
The Outreach and Enrollment Specialist is responsible for the coordination of all health coverage programs and providing access to these programs for the patients of School Health Clinics of Santa Clara County.
Incumbents in this position will be expected to demonstrate a commitment to working with families living in disadvantaged communities and from diverse racial and ethnic backgrounds.
The Outreach and Enrollment position will be part of a multi-disciplinary team comprised of primary care providers, the Care Coordinator, and the Clinic Manager.
In collaboration with this team, this position provides outreach for patients and provides coordination to a variety of support systems to patients including care coordination and care management services to School Health Clinics of Santa Clara County (SHCSCC) patients.
*PRINCIPAL DUTIES AND RESPONSIBILITIES:* * Provides enrollment assistance (including but not limited to completing coverage applications, gathering the required documentation and troubleshooting the enrollment process) for uninsured children and adults to access subsidized, low-cost and free health insurance programs through the health insurance marketplace, Cal-Win Medi-Cal, and the Primary Care Access Program (PCAP) * Provide structured patient education on health coverage, engage in follow-up conversations and offer renewal assistance for enrolled individuals.
* Distribute outreach materials to patients, community members, partner organizations and businesses to build coverage option awareness.
* Attend community events in order to promote coverage options and the mission and services of the organization.
This may require evening and/or weekend hours.
* Collaborate with various organizations to build awareness of coverage options, enrollment and build referral linkages.
* Attend and successfully complete all required training programs; participate in ongoing conference calls, webinars, and other professional development opportunities.
* Strive to meet project thresholds and goals set by funding resources and accurately provide required reporting to track goal achievement and client satisfaction.
* Assist in the development and implementation of organizational outreach and enrollment initiatives.
* Other duties as assigned.
*EXPERIENCE AND TRAINING:* * Bachelor’s Degree preferred.
* English-Spanish preferred.
* Experience in planning and implementing projects and coordination of functions and setting goals and meeting timelines promptly.
* Comply with all applicable federal and state training requirements related to the development of expertise in eligibility, enrollment, and program specifications.
* Able to work accurately, independently and as part of a team.
*MINIMUM KNOWLEDGE AND ABILITY:* * Knowledge of health insurance programs and public coverage options.
* Knowledge of childhood obesity, exercise, and nutrition.
* Ability to establish and maintain productive working relationships with community agencies, diverse populations, schools, and faith-based groups.
* Ability to communicate effectively with families, groups and actively engage the general public in community outreach.
* Ability to work independently, prioritize, problem solve and exercise good judgment with minimal supervision.
* Ability to work with diverse populations.
* Computer literate with in-depth knowledge, experience, and computer skills with knowledge of operations of word processing and database.
* Ability to speak, write and understand Spanish required.
*WORKING CONDITIONS:* * Normal office environment with some exposure to excessive noise, dust, temperature and the like.
* Combination of regular office environments with frequent travel to clinic sites and community meetings/events and schools.
* When working at various sites, may be exposed to communicable diseases.
* Frequent interruptions in work to answer the telephone and respond to urgent requests.
* Local travel required.
* Must have a car available for daily use, a valid California driver’s license and auto insurance as required by the State of California.
