*Hospice Clinical Liaison Job Responsibilities - *This Clinical Liaison will be in charge of leading the operations in the Santa Clara County Region.
This person will be a integral part of our team, ensuring we meet the needs of our patients.
Duties will mainly include partnering with the community (hospitals, skilled nursing facilities, and families) to fulfill their Hospice needs.
This person will use their clinical expertise to liaise between our internal clinical team and the community, and then also help onboard / visit patients.

Pay: From $80,000.
